Q: What is the primary specialty of Sanford Healthcare?
We specialize in the unique field of psychiatric and memory care home health nursing.
Q: How should I contact Sanford Healthcare to determine if consulting services will benefit me?
You can contact us by email or by phone, and we will work with you to develop a package that suits the unique needs of your agency.
Q: Are the educational materials, promotional materials, presentations or any other products offered through consulting services at Sanford Healthcare previously formulated?
No. Every project taken on by Sanford Healthcare is unique and fully customized to our particular customer’s requests and budget.
Q: What happens after we have chosen to work with Sanford Healthcare?
After a discussion about your needs and budget, SHS then will then send a contract to the Agency along with an invoice for 50% of the consulting fee. Travel fees can be estimated and included in the invoice. The balance is payable within 30-days of the service provided (SHS will send an invoice).
Q: Are you able to connect us with referral sources?
No. Because we serve multiple home health agencies, we are unable to endorse any particular agency. We are able to train marketing teams how to promote our programs effectively in your area.

Q: Can you educate our staff on the uses for psychiatric home health nursing?
Yes. We provide consulting services relating to starting and maintaining a successful psychiatric home health nursing program. We also can provide State of Florida - approved 2 hour Alzheimer’s class for your direct-care staff.
Q: Can you assist us in making sure our psychiatric program is financially sound and within Medicare and state guidelines?
Yes, we are also able to provide consulting services concerning those issues. We can also perform a mock audit or a financial review of your existing program and provide suggestions for improvement and growth.
Q: Will you ever share our referral sources or information about our agency with our competitors?
No. Sanford Healthcare Solutions operates under a strict code of confidentiality. We do not share any proprietary information amongst our clients.
Q: Can you create a memory loss program for our agency?
Yes. We can create a unique Memory Care Program for your agency. We have been able to provide this service in the past, and are able to do this on a consultant basis. It is a requirement in some states, including Florida, that any HHA that claims to have a Memory Care program must have supporting materials.
Q: Are you able to be available for our company on an as-needed basis to answer questions or provide support for our psychiatric and memory care programs when we need it?
Yes. Clients who need professional counsel from time to time can access services by retaining the Consultant for a reasonable fee. This retainer is billed against only when you access services, and a detailed statement is sent to the customer each month.

Q: What geographical area do you cover?
We will cover psychiatric nursing and social worker visits in the following counties: St. Johns, Duval, Clay, Flagler and Volusia. We may do visits in other counties on a case by case basis.
Q: How do we assign a psychiatric patient to a Sanford Healthcare nurse?
You will need to contact us to obtain basic HR information. After this initial setup, you can call us anytime Monday-Friday from 8-5 to request a psychiatric evaluation on a patient. You may fax any pertinent patient information to us. We can usually see most patients in the Jacksonville area within 48 hours.
Q: How will we receive your visit notes?
Visit notes can be sent same-day by scan, fax or mail, and in some cases, may be hand-delivered.
Q: Do we have to create a psych nursing visit note?
No. We have a standard psychiatric visit note form that we use, and that is paid for by Sanford Healthcare. If you already have your own note, we will be glad to use that instead.
Q: Do you do OASIS?
We do not. AHCA regulations state that HHA’s should have direct employees complete all OASIS data collection.
Q: How will your nurse communicate with us?
After completing the initial evaluation, our nurse will call your clinical manager or designee with report and visit frequency. We will remain in communication with your agency regarding any further patient needs.
Q: What type of patients can the psych nurses see?
Patients appropriate for psychiatric home health nursing can include homebound patients with unstable Axis I psychiatric diagnoses, or patients diagnosed with any type of dementia.
Q: What does a psychiatric home health nurse do?
A psychiatric home health nurse can assess and educate a patient or caregiver when unstable psychiatric illness is present. The nurse can perform various standardized tests to determine degree of memory loss, appropriate level of care, or depression. These results can be reported to the physician to assist in care planning for the patient. Psychiatric nurses can assess medication compliance, and alert caregivers and medical team if signs of psychiatric decline are present. Psychiatric home health nurses are often able to decrease hospitalizations in chronic patients.
For Alzheimer’s patients, the nurse will create a teaching plan to educate caregivers on safe and effective care for their loved one. The emphasis will be on home safety, memory loss strategies, and dealing with problem behaviors.

Holly Sanford, RN, BSN-BC
Holly Sanford is Sanford Healthcare’s founder and CEO and is responsible for leading Sanford Healthcare’s clinical and administrative staff, and for providing educational and professional consulting services.
Since 2006, Holly has worked in the unique field of psychiatric home health care, starting as a field nurse, advancing to Directorship of a large psychiatric home health program, and eventually becoming a home health agency Administrator. During this time, she also served as a consultant, serving group homes for the developmentally disabled and authoring a memory care program for home health. Prior to that, Holly was an inpatient psychiatric RN, caring for both children and adults with severe mental illness.
During more than 12 years in nursing, she has participated in many facets of psychiatric care, including leading inpatient therapy groups and community support groups. She currently volunteers in her community facilitating an Alzheimer’s Caregiver support group for the Alzheimer’s Association and a local grief support group in her hometown.
Throughout her career, Holly has been committed to the advancement and availability of psychiatric care to her community. She is approved by the Department of Elder Affairs as an Alzheimer’s disease and related dementias educator, and has been honored as one of “The Great 100 Nurses of Northeast Florida”.
Holly Sanford founded Sanford Healthcare Solutions in 2010 with a vision to make psychiatric home health care available and accessible to all patients who would benefit. She hopes to do this by educating home health providers on all aspects of this valuable program. She holds a bachelor's degree with a major in Nursing from Liberty University.
